B.Z. Service Station Maintenance, Inc.
1041 Triangle Ct/P.O. Box 933 (Zip 9569 1)
West Sacramento, California 95605
Phone (916) 371-2380
Fax (916) 371-2540 RECEIVED
March 1, 2017
Lic. #433159
JAN 6 J 201-1
Country market Place ENWRUNMENTAL HEALT[i
1789 Charter Way
E'FRMIrIgRVICES
Stockton
Hello;
Quote to repair penetration boots in UDC's 516, 9/10, 11112 and repair the leak in the 91
annular- $37,500.00.
Scope of work
-pull the three dispensers and loosen up the impact and vapor valves
-with in the UDC remove the old penetrations and clean off all the old adhesive
-install the new custom made repair penetration fittings and glue them in place
-excavate the top of the 91 tank to inspect the annular. Assuming the annular leak is at the
junction of the riser and the tank. Those are Trusco tanks and they would send out some
2" fiberglass pipe to be glued into the opening on the tank for the annular space. The
tanks came with a gauge in them that was held in with a type of putty so I am hoping the
original installer didn't clean all the old putty and that's where the problem is.
-If the annular leak is not found at the above mentioned fittings, there may be additional
costs to further locate the leak on a time and materials basis
Quote includes
-initial permit fee
-parts, tax
-diagnostic testing
-post construction testing
Required deposit to order parts $10,000.00
Payments as follows:
30 days from completion: $5000.00
60 days from completion: $5000.00
90 days from completion: $5000.00
120 days from completion: $5000.00
150 days from completion: $5000.00
180 days from completion: $5000.00
210 days from completion: $2500.00
LIMITATION OF LIABILITY FOR DAMAGES: It is expressly agreed that BZ
Service Station Maintenance shall not be responsible for any indirect, accidental or
consequential damages whatsoever arising from any install or repairs made to the above
