This information was required to be submitted by April 06, 2017 . This <br /> information has not been received resulting in a non-compliant status for this facility. An operator that receives an <br /> inspection report shall have 30 days to submit a written response that includes a statement documenting corrective <br /> actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting <br /> corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. <br /> This is a repeat violation, Class II. <br /> 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. <br /> Maintenance and monitoring records for the last three years were not found on site. The 2016 and 2017 monitoring <br /> certification reports and the 2015 secondary containment report were not found on site. These records shall be <br /> maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or <br /> sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4) <br /> results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring <br /> equipment, ground water elevations, or other test results; (6)results of inventory readings and reconciliations. <br /> Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit <br /> copies to the EHD. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 319 CCR 2635(b)No spill bucket,failed to contain five gallons, or failed to provide a means to keep it empty. <br /> The 87 octane direct-bury spill container failed when tested. All spill containers shall have a minimum capacity of <br /> five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately discontinue <br /> deposition of petroleum into this tank until the component is repaired or replaced by a properly licensed,trained, and <br /> certified contractor under permit and inspection of the EHD. If the spill container can't be replaced immediately, <br /> there is a possibility that the 87 octane UST system may be red tagged to prevent fuel inputs. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by April 26, 2018 . <br /> Note: The product lines for Dispensers 3/4, 7/8 and 11/12 were disconnected at the STP sump at time of testing <br /> and the dispensers were removed. UDC monitoring for the mentioned dispensers has been ongoing during the <br /> retrofit. Sensors at these UDC's underwent testing and passed. Line leak detectors were tested but only <br /> dispensers 9/10, 5/6 and 1/2 had product lines operational.
