The following is an itemized list of underground storage tank violations that have not been addressed for COUNTRY MARKETPLACE as of October04, 2016. Open violations from March 08, 2016 inspection Violation #103 -Current financial responsibility documents not submitted. Financial responsibility documents have not been submitted to the EHD. The financial responsibility documents submitted for 2015 were actually copies of the 2013 documents with the dates changed and no new, current signatures. Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. Violation #109 - Failed to test secondary containment. Secondary containment testing was fast performed on July 16, 2012 and was due by July 2015. Testing has not been completed for all components and is 8 months past due. According to the test results from July 14, 2015, the 87 north piping line and the UDC 11112 were not tested. Secondary containment testing is required once every 36 months. Immediately schedule this test and provide 48 hours notification to the EHD. Violation #201 - Failed to maintain alarm logs and/or records of follow up actions. Maintenance and monitoring records for the last three years were not found on site. The 2015 monitoring certification test results were not found on site. Maintenance records for work done in the last year was not found on site, including work orders for the secondary containment testing done on July 14, 2015 and the response to an L1 91 annular alarm on January 16, 2016. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results, (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies of the missing records to the EHD. Violation #314 - Secondary containment not tight. The July 14, 2015 secondary containment test results indicate a failure at the 91 annular, UDC 5/6, and UDC 9110. All secondary containment for the UST system must be tight. Immediately have a properly licensed, trained; and certified contractor repair or replace the failed component under permit and inspection of the EHD. Page 1 of 1
