UST Facility Operating Permit is not current in CERS. The California Department of Tax and Fee Administration (formerly Board of Equalization or BOE)number TK STF 44024646 is closed and no longer active. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a Class II violation. 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. Maintenance and monitoring records for the last three years were not found on site. Monitoring System Certification reports for 2015, 2016 and 2017 were not found onsite. These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment, ground water elevations,or other test results; (6)results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a minor violation. 306 CCR 2630(d)Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. The 91 STP sump sensor was raised and found to be between three to four feet above the bottom of the sump and not located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. The sensor was relocated to the lowest point of the sump by the service technician. Ensure that all monitoring equipment is maintained to detect a leak at the earliest opportunity. This is a Class II violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork by April 29, 2018 . Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer(if applicable), and the Owner Statement of Designated Operator Compliance. The UST Owner/Operator: Written Agreement, if applicable, can be stored at the facility or uploaded into CERS.
