The following is an itemized list of underground storage tank violations that have not been addressed for MORADA CHEVRON FAST N EASY #60* as of March 11, 2017. Open violations from March 11, 2017 inspection Violation #103 -Current financial responsibility documents not submitted. Financial responsibility documents have not been submitted to CERS within the past 365 days. The CFO letter is greater than 365 days old. Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. Violation #104-UST operating permit application for facility and tank information not submitted or current. The UST tank information forms for all of the tanks in the California Environmental Reporting System (CERS) indicates"no"for the striker plate. The tanks all have striker plates under the fill pipes. Correct the tank forms in CERS and submit the corrections. Open violations from April 25, 2016 inspection Violation #201 -Failed to maintain alarm logs and/or records of follow up actions. Maintenance and monitoring records for the last three years were not found on site. The April 2015 secondary containment test results were not found onsite. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records, (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. Violation#209 - Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to document all the alarms from the attached alarm history on the November 2015 designated operator monthly inspection report and failed to check that they were responded to appropriately. The missing alarms include: An 1-2: 91-2 STP Sump Fuel Alarm on November 16, 2015. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. The designated operator failed to check and note the correct test dates for the Monitoring System Certification, Leak Detectors and Spill container test on the August 2015 through April 2016 and the May 2015 through June 2015 designated operator monthly inspection reports. The test date listed on the inspection reports is April 21, 2015. The correct test date on file is April 22, 2015. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done.
