Contact the local fire department and San Joaquin Valley Air Pollution Control District for <br /> any applicable permits. <br /> 2. Any changes shall be submitted to this office for approval prior to commencing work. All <br /> new addenda supercede the previous ones. <br /> 3. At least one person who holds the International Code Council (ICC) UST Service Technician <br /> certification and manufacturers' certificate of training for all components installed or tested <br /> must be onsite when work is performed. <br /> 4. Each person installing or testing any component must hold a certificate of training from the <br /> manufacturer of that component. <br /> 5. Contact EHD for the following inspections at least 48 hours in advance: <br /> a. Testing of new line leak detector <br /> 6. Complete and submit a copy of a UST Installation Certificate of Compliance(Former Form <br /> C) for the 87 octane tank no later than 30 days after work is completed. <br /> 7.
