The permit has been approved with the following conditions: <br /> 1. Contact the local fire department,and the San Joaquin Valley Air Pollution Control District for <br /> applicable permits and requirements. <br /> 2. Any changes in information or scope of work must be submitted to SJCEHD prior to commencing <br /> work. <br /> 3. At least one person who holds the International Code Council"UST Installer/Retrofitter"and <br /> "UST Service Technician"certification must be onsite AT ALL TIMES when work is performed. <br /> 4. Manufacturer training certificates must also be presented at time of inspection prior to any testing <br /> taking place. <br /> 5. If concrete is broken over the tank you must arrange the following inspection appointments: <br /> a. Corrosion protection:prior to backfilling around the spill buckets,corrosion protection of the <br /> risers must be verified. <br /> b. Overfill prevention:overfill prevention system in compliance with CCR 2635 (b)(2)must be <br /> verified. <br /> c. Spill Bucket:after concrete is poured,verify minimum 5-gallon capacity and conduct a 30-minute <br /> lake test in compliance with CCR 2635(b)(1). Proper drainage must also be verified. <br /> d. Third party tank integrity tests must be performed once concrete is poured,prior to the <br /> contractor signing off of the jobsite. The test results must be submitted to SJCEHD within 7 days <br /> of completing this project. <br /> e. Leak Detector Test: leak detector must be tested to confirm that the device detects a leak of 3 <br /> gallons per hour at 10 psi. <br /> L Sensor Testing: Sensor must be tested in water to verify functionality. <br /> 5. If dispenser pans are replaced,soil samples are to be collected and analyzed as indicated in the <br /> minimum verification analysis. Samples must be taken under each removed dispenser and every 20 <br /> feet of piping.Analytical result shall be submitted to Environmental Health Department(EHD) <br /> directly from the lab.Additionally,EHD must be contacted for the following inspections: <br /> a. Soil sampling below each dispenser/piping. <br /> b. 30-minute pressure test on primary and secondary lines. <br /> c. Soap test on primary and secondary lines. <br /> d. 24-hour lake test. <br /> e. Final inspection. <br /> 6. Contact Lori Luces,EHS at(209)468-0341 for inspection appointments.48-hour notice for inspection <br /> appointments is required. <br /> 7. Permit expires 90 days from the approval date.
