RECEIVED <br /> CORRECTIVE ACTIONS MAR 16 2017 <br /> ENVIRONMENTAL HEALTH <br /> #105: The required changes for the leak sensor modEPIPfrTiMENT <br /> 407 to 409 have been made. Also, we have included the <br /> monitoring plan for all the USTs separately even though it is <br /> identical for all USTs. We are attaching the copy for the <br /> department review. <br /> #107: UST monitoring site plan has been updated as required <br /> and we are attaching a copy of the updated version which <br /> shows sensor under dispenser 3-4 and correct leak detector <br /> (99LD-2000). <br /> #111: Reason for the delay was due to confusion over last year <br /> testing dates, but going forward with the advice of D.O. we will <br /> make sure that we have correct testing dates and it must be <br /> done in a timely manner. <br /> #112: As we mentioned above, we will take all necessary steps <br /> to perform testing on time. <br /> #113: We are sorry that your office had not received test report <br /> within 30 days. We already talked to service provider to <br /> prevent this in the future. <br /> #201: We hereby attaching the copy of Annual monitoring <br /> system certification report for 11/25/2014 testing. We also <br /> attached the technician visit to site on 08/22/16 and 01/23/17 <br /> when C1 and L1 alarm was cleared.
