Hwy 99, Stockton, CA 95215 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the application for a permit to replace the master turbine control board for the 87 tank on the <br /> underground storage tank (UST) system at the above noted address. The permit has been <br /> approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Stacy Rivera at (209) 468-3440 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). Please be <br /> aware that inspections for emergency work must be scheduled within five days of permit <br /> approval. <br /> a. Verification of positive shutdown. <br /> b. Leak detector test: Leak detector must be tested to confirm that the device can <br /> detect a leak of 3 gallons per hour at 10 psi for both 87 turbines. <br /> 4. If concrete will be broken near the UST and/or piping, a secondary containment test <br /> must be performed on the affected areas once concrete is poured in accordance with the <br /> manufacturer's guidelines. <br /> 5. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible <br /> valid, current contractor's licenses issued by the Contractors State License Board <br /> (may be issued to the installation company), or a Tank Tester's license issued to the <br /> individual by the State Water Board, and <br />
