The facility Operating permit application -- Id has been corrected in CERS and <br /> is now listed as FA0003747. <br /> UST Tank Information Forms: The following items were reviewed and updated as required in <br /> CERS - Tank Id, piping sump, vent and vapor recovery piping construction, piping/turbine <br /> containment, vent piping transition sump, and fill components. <br /> 106—Corrected. The Monitoring and response plans were reviewed and the Pipe monitoring and <br /> personnel responsibilities were updated as required CERS. <br /> 204— Corrected. The Current monitoring plan is available for inspection in CERS. This copy can <br /> be accessed from the facility via internet. In addition, a printed copy of the HMBP, including the <br /> monitoring plan, is kept at the facility. <br /> 301 — Corrected. The overfill mechanism (float sensor) was replaced and re-tested on 10/24/14 <br /> by TAIT Environmental. A permit was obtained from SJ County EHD prior to the work. An <br /> inspector(Stacy Rivera)was on site to witness the test. A report of this work will be submitted to <br /> the CUPA within 30 days as required. <br /> 326 — Corrected. As described for item 301 above, the overfill mechanism was replaced and <br /> retested as required. <br /> Overall Inspection Comment: regarding the sensor in the piping sump which was not tested on <br /> 9/29/14, this sensor was tested by TAIT on 10/24/14, and passed. A report will be submitted as <br /> required. <br /> Large Quantity Hazardous Waste Generator Inspection Report(dated 9129/14): <br /> 107- This item in the inspection report stated the Contingency plan incomplete, 'Upon review of <br /> the plans provided at the time of inspection a listed emergency coordinator could not be found'. <br /> Immediately prepare or amend an existing plan to fulfill the requirements of this chapter. Submit <br /> a copy of the updated portion of the contingency plan to the EHD. <br />
