SAN JDANIN COUNTY— Environmental Health Department Underground Storage Tank Program Inspection Report Facility Name: Facility Address: Date: YRC INC 1535 E Pescadero Ave, Tracy February 01, 2018 SUMMARY OF VIOLATIONS (CLASS I, CLASS II, or MINOR - Notice to Comply) Item # Remarks 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. Site map is lacking general piping layout and tank #4 annular sensor was observed during inspection to be a 303 sensor. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. This is a repeat violation, Class II. 211 CCR 2715(f)(2) Designated operator employee training not performed or log not kept. The designated operator employee training for 2017 was performed but missing the hire dates for all employees listed. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator and maintain the list on site. The designated operator on site during the inspection corrected the hired date and included all hire dates after July 1, 2005. This was corrected on site. This is a repeat violation, Class II. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 3/3/18. Documents reviewed by regulator: Employee training log, designated operator monthly inspection reports, monitoring plans, response plan, financial responsibility, owner/operator agreement, monitoring system certification testing reports (2015, 2016, 2017), secondary containment testing reports (2016), operating permit, alarm histories and alarm setup, maintenance records. Documents received by operator: Routine inspection report and return to compliance certification form. AFTER THE COMPLIANCE DATE, EHD WILL BILL FOR ALL TIME AND ACTIVITIES ASSOCIATED WITH BRINGING THIS FACILITY BACK INTO COMPLIANCE.
