The designated operator failed to check and note the correct test dates for the secondary <br /> containment test on the May 2012 through April 2013 designated operator monthly inspection reports. <br /> The test date was listed as October 21, 2011, but should have been listed as October 12, 2011. The <br /> designated operator failed to check and note the correct test dates for the cathodic protection test on the <br /> January 2013 through April 2013 designated operator monthly inspection reports. The test date was <br /> listed as June 11, 2012, but should have been listed as May 11, 2012. During the monthly inspection, <br /> the designated operator shall check that all required testing and maintenance for the UST system have <br /> been completed and document the dates they were done. Ensure that designated operator performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> 303. The main red dye diesel turbine sump sensor was at the bottom of the sump approximately five <br /> inches away from the lowest point of the sump, part way up the sump saddle, and not located to detect a <br /> leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak at <br /> the earliest possible opportunity. Take the necessary precautions to ensure that all sensors are located <br /> at the lowest point to detect a leak at the earliest opportunity. <br /> 314. The drop tube with mechanical flapper set at 95% for the overfill prevention system is missing from <br /> the riser of the red dye diesel main UST. Immediately discontinue deposition of petroleum into this tank <br /> until the component is repaired or replaced by a properly licensed, trained, and certified contractor under <br /> permit and inspection of the EHD. If the failed component can't be replaced immediately, there is a <br /> possibility that the red dye diesel main UST system may be red tagged to prevent fuel inputs. <br /> Note: The swivel adapter from the red dye diesel main tank riser was switched to the red dye diesel <br /> siphon tank riser. The removed swivel adapter was replaced with a four inch cap to prevent filling of the <br /> red dye diesel main tank riser. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by May 4, 2013. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Inspedoc R.i Title: <br /> EHD 23-02 Rev 04/19/12 Page 3 UST INSPECTION REPORT <br />
