Return to Compliance <br /> To: San Joaquin County Environmental Health Department <br /> I'Sam Abundis from Trans-System,Inc. am writing a statement to why a <br /> fulel alarm was not logged in my daily inspection report on May 12, 2010. <br /> The reason being is that I Sam Abundis had logged in all functions <br /> normal in the morning when I stepped in, on that Wednesday morning <br /> the 121h of May. <br /> I had my mechanic inspect the fuel pumps for any leaks, rain water, dirt <br /> or debris ... while inspecting the sump my mechanic accidentally tripped <br /> the sensor therefore causing the 1:1 pipe sump alarm to go off at about <br /> 12:40 pm. At that time I was currently at lunch so I did not hear about the <br /> alarm until after I returned from lunch, at that time my mechanic <br /> informed me on the incident. <br /> He gave me the fuel alarm receipt and I placed it on my desk so that I <br /> cold make the correction on my daily log , being that there were no real <br /> problems with the sump pump and it's sensor, changing the log slipped <br /> my mind and therefore the log was never changed to fuel alarm, it was <br /> brought to my attention by the inspector on Monday when we had our <br /> annual testing on site I then showed her the receipt and told her I did have <br /> the fuel alarm receipt but forgot to change it on the log it is now stapled to <br /> the daily monitoring records and it is logged correctly I will inform our <br /> designated operator Dan Maculrath when he comes in to do our <br /> monthly(May) inspection so that he is also aware of the incident thank <br /> yo • <br /> Sincerely <br /> Sam Abundis <br /> May 19, 2010 Page 1 <br />
