SAN JOAQUIN <br />COUNTY <br />Environmental Health Department <br />Underground Storage Tank Program Inspection Report <br />Facility Name: <br />Facility Address: <br />Date: <br />ORLANDOS <br />18754 E HWY 26, LINDEN <br />July 10, 2018 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />201 <br />CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. <br />The July 2015 and July 2017 annual monitoring system certification test results were not found onsite. These <br />records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all <br />monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual <br />observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other <br />monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and <br />reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on <br />site and submit copies to the EHD. <br />This is a repeat violation, Class ll. <br />211 <br />CCR 2715(f)(2) Designated operator employee training not performed or log not kept. <br />The designated operator employee training for Mikayla Hoyle, Miranda McArthur, Jocelyn Canela, Debbie D., and <br />Fernando Oseguera is not current. The training log contained an arrow stemming from the training date of 03/05/17. <br />The designated operator shall train facility employees for which he or she is responsible in the proper operation and <br />maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br />the training records to the EHD. <br />This is a repeat violation, Class ll. <br />320 <br />CCR 2635(b)(2) Overfill prevention system not present or failed to meet overfill requirements. <br />The automatic tank gauge (ATG) system failed to activate a visual alarm when tested. <br />All underground storage tanks shall be equipped with an overfill prevention system that does not allow for manual <br />override and meets one of the following requirements: <br />1. Alert the transfer operator when the tank is 90 percent full by restricting the flow into the tank or triggering an <br />audible and visual alarm; or <br />2. Restrict delivery of flow to the tank at least 30 minutes before the tank overfills, provided the restriction occurs <br />when the tank is filled to no more than 95 percent of capacity; and activate an audible alarm at least five minutes <br />before the tank overfills; or <br />3. Provide positive shut-off of flow to the tank when the tank is filled to no more than 95 percent of capacity; or, <br />4. Provide positive shut-off of flow to the tank so that none of the fittings located on the top of the tank are <br />exposed to product due to overfilling. <br />The light bulb was replaced and retested during the inspection. <br />This was corrected on site. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />FA0007722 PR0507164 SCO01 07/10/2015 <br />EHD 23-01 Rev. 10/31/2017 Page 4 of 5 Underground Storage Tank Program OIR <br />1868 E.
