• responsibility of the SHO in coordination with the project Safety and Health Officer and <br /> Project Manager. <br /> The following is a listing of general work practices which must be complied with order to <br /> ensure the greatest degree of safety at the work site: <br /> • All personnel working at the site and all visitors to the site are required to read this Site- <br /> specific Safety and Health Plan and to sign the Acknowledgment of Understanding <br /> before they may enter any exclusion zone. In addition, any person working at the site <br /> or visiting the site must sign the Site Entry Log daily (Attachment 4). <br /> • Eating, smoking, drinking or chewing gum or tobacco are prohibited except in the <br /> support zone. <br /> • Hands and face must be washed and dried before exiting the decontamination zone. <br /> • Contact with contaminated surfaces or materials should be avoided. Personnel will <br /> avoid walking through puddles, mud, excavation piles or discolored areas and will not <br /> sit or kneel on the ground. <br /> • Protective clothing and equipment will be worn at the work site at the protective level <br /> • specified by the Safety and Health Officer. <br /> • Facial hair which may interfere with proper respirator fit will not be allowed in the <br /> exclusion zone. <br /> • Personnel will typically dispose of expendable Personal Protective Equipment (PPE) <br /> into approved containers immediately after removal of such items. <br /> • All personnel must use the buddy system at all times while in the exclusion zone. <br /> Under no circumstances shall employees work alone onsite. <br /> • Equipment will be kept in proper working order, free of accumulated lubricants, <br /> contaminants, or other hazardous or flammable substances. <br /> • No containers of fuels or other flammables will be kept within 100 feet of any <br /> excavation and loading operations. <br /> • Daily safety briefings will be held by the SHO. <br /> • Policies promulgated in this Site-specific Safety and Health Plan will be followed by all <br /> employees. Changes in any procedures or policies contained in this plan will only be <br /> implemented after approval from the project Safety and Health Officer. <br /> • <br /> C-11 <br />
