The light bulb used as part of the visual overfill prevention system was not currently functional on <br /> the UST. The light bulb was replaced, retested during the inspection and passed. <br /> 204. The designated operator failed to document all the alarms from the attached alarm history on the <br /> August and February 2012 designated operator monthly inspection reports and failed to check that they <br /> were responded to appropriately.
