No permit extension request will be <br /> allowed after the December 2012 date. No permit fees or addition application needs to be <br /> submitted for this extension. <br /> If the operator decides to perform UST retrofit after the December 2012 date, new application <br /> and permit fees must be resubmitted. <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the application for a permit to retrofit manways for prevention of water intrusions by removing <br /> existing steel manway lids and installing new manway lids, new Phil-Tite 5 gallon spill buckets, <br /> EBW Autol-imiter II shut-off valves, vapor swivel adaptor and fill swivel adaptors along with new <br /> Blueline sump penetrations on the underground storage tank (UST) system at the above noted <br /> address. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Thuy Tran at (209) 468-8257 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Spill bucket test: Verify minimum 5-gallon capacity and pass a one hour hydrostatic <br /> test (or equivalent). Proper drainage must also be verified. <br /> b. Secondary containment testing on 87, 89, and 91 USTs. <br /> c. 24-hour hydrostatic test (or equivalent) on 87, 89, and 91 Fill and STP sumps if any <br /> penetrations are installed. <br /> d. Overfill prevention verification: Measurement of drop tubes to verify 95% tank volume <br /> shut off. <br />
