W. Turner Road, Lodi. <br /> I have reviewed the information you submitted in response to the memo of deficiencies I wrote to <br /> you on May 5, 1998. The following items still need to be submitted to this office before the <br /> applications can be approved. <br /> INSTALLATION <br /> I. The owner needs to complete the financial responsibility information and submit a letter from <br /> the chief financial officer(see enclosed example). <br /> 2. You still need to submit anchorage calculations stamped by a California registered engineer <br /> or architect to show that the underground storage tanks will not be subject to floatation. <br /> 3. You still need to submit product specification sheets for the proposed Ameron LCX piping <br /> with an approval from UL or ULC on the sheet. <br /> 4. You still need to submit evidence of training, by a certificate of training issued by the tank and <br /> piping manufacturers, for the employees who will be installing the tanks and piping. <br /> 5. You still need to submit a cross section of the 12,000 gallon tank illustrating the piping <br /> including secondary containment, overflow protection equipment, and extensions of all pipes <br /> and/or ports from below grade to finish grade(i.e. similar to the illustration of the tank on page <br /> three of the blue prints you already submitted). <br /> 6. You still need to illustrate striker plates below all accessible openings of the tanks on the <br /> cross sectional diagram required in item number four of this memo. <br /> 7. You still need to submit an illustration of the detail below the dispensers including the product <br /> piping, dispenser containment, flex boot connectors etc. <br /> 8. You have identified a model number 970-xxx for the monitoring probe to be installed in the <br /> annular space of the tank. You must choose an approved probe listed in "LG-113-12", list of <br /> leak detection equipment and methods for underground storage tanks, written be the State of <br /> California State Water Resources Control Board (SWRCB). Please choose an approved <br /> probe or submit an approval letter for use of this probe by the SWRCB. <br /> A Division of San Joaquin County Health Care Services <br />
