A new designated operator, Philip De Forge was hired on 2-16-12 and notification was not provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD within 30 days. Submit notification to the EHD identifying all the designated operators for this facility by 5-25-12. During the inspection, Paul Perotti provided a current copy of the owner designated operator statement. Corrected on site. 204. The designated operator failed to check and note the correct test dates for the line test on the May 2011 through April 2012 designated operator monthly inspection reports. The designated operator reports from May 2011 through January 2012 listed the annual line test as 5-17-10. It should have been listed as 4-29-11. The designated operator reports February 2012 through April 2012 list the line test as not applicable and left the date blank. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. The EHD will not accept the monitoring system certification testing performed at 9:00 AM on 4-25-12. The EHD received a notification on 2-22-12 that the monitoring system certification and spill container testing would be performed at 1:00 PM. The EHD inspector was not able to witness the testing nor conduct a complete inspection. A 48 hour notification must be provided to the EHD for a properly licensed, trained, and certified contractor to perform the monitoring system certification testing which can be witnessed by the EHD inspector. The monitoring system certification, spill continer, and annual product line integrity test is due before May 2012, otherwise it will be considered late and additional violations will apply. A reinspection will be billed at the EHD's current hourly rate. Complete and submit a copy of the Return to Compliance Certification form with a statement of how each violation was corrected, including supporting documentation, by 5-25-12.
