Turner Rd., Lodi, CA 95242 <br /> The San Joaquin County Environmental Health Department(EHD) has completed the <br /> review of the application for permit to conduct a monitoring system upgrade at the above <br /> noted address. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution <br /> Control District for any applicable permits and requirements. <br /> 2. Contact Toua Yang at(209)468-3459 to arrange the following inspections <br /> appointments (a 48-hour notice is required for inspection appointment(s): <br /> a. EHD must witness the functionality of the monitoring system to ensure <br /> that monitoring has been restored to meet the requirements of California <br /> Code of Regulations Title 23 and the Health & Safety Code. <br /> 3. The underground storage tank monitoring plan shall be updated to address this <br /> facility's monitoring. <br /> 4. Before the existing monitoring console's alarm history is cleared, a current <br /> printout must be created and available for EHD inspection. Inspection may occur <br /> during the functionality test. <br /> 5. Any changes in information and scope of work must be submitted to EHD for <br /> approval prior to commencing work. All new addenda supersede the previous <br /> ones. <br /> 6. All components installed must be certified by an independent testing organization <br /> and bear the appropriate mark as required by the independent testing organization. <br /> 7. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board (maybe issued to the installation <br /> company), and <br />
