As part of the inspection, you listed three items that required
immediate attention. Below is my response for each item:
1. A copy of the Certification of Financial Responsibility and Certificate of Insurance for
2007 was given to you at the time of the inspection on May 21, 2007,
2. The liquid observed in the north piping sump was removed by the company performing
the underground storage tank testing, CPT, Inc. This material was removed with other
liquid that was used in the testing process.
3. In regards to the two missing Monthly Visual Inspection Reports, below is an
explanation for each month:
August 2006 - Due to changes in the company, the vendor contract with Petroleum
Engineering was cancelled. It was cancelled because the responsibility was assumed by
the Verizonbusiness Facilities Engineering Group (this was the transition period). The
inspection was resumed in September 2006 by Verizonbusiness Engineer Chris Huffman.
October 2006 - The October inspection was missed because the Designated Operator
Verizonbusiness Engineer Chris Huffman went on disability. He was then replaced by
Gregg Effisimo (Verizonbusiness Engineer) in November, and eventually the current
Designated Operator Robert Guldseth.
4. Three facility employees were trained by the Designated Operator, Robert Guldseth, on
June 14, 2007. A copy of the training record has been enclosed.
