State law provides <br /> for a -fee system -to cover -the cost of implementing this state -mandated <br /> Loca). fees (see attached Fee cc /euul for th .se required inspectional <br /> services, will be billed on a yearlyl basis. The facilities Permit to Operate <br /> will. be issued for a five year perio In _--tddition to the yearly <br /> inspectional fee, this statement wil I also include a $S6/tank. State surcharge <br /> fee. The State surcharge fee per tank will be charged every five years or <br /> whenever the facilities permit is re I newed or amended. All State surcharge <br /> fees will be transmitted by the Healith District to the California Water <br /> Resources Control Board. <br /> To apply for a facilities Permit to loperate, complete the attached Fee ' <br /> Worksheet and submit the appropriat fees.
