Beginning <br /> January 1, 2014, all businesses are required to submit all new(or any changes to existing) UST information, including: <br /> UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Owner/Operator <br /> Written Agreement(if applicable), UST Letter from Chief Financial Officer, and Owner Statement of Designated UST <br /> Operator Compliance online to the CERS at http://cers.calepa.ca.gov. Be sure to include any other relevant activities <br /> and required fields. <br /> This is a repeat violation, Class II. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> Alarm logs for 2015 were not found on site. These records shall be maintained on site for at least three years. <br /> Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and <br /> maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the <br /> field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) <br /> results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring <br /> records for the last three years on site and submit copies of 2015 alarm logs to EHD. <br /> This is a minor violation. <br /> 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br /> An approved and current copy of the monitoring plans were not available for inspection. The monitoring plans provided <br /> on site listed the mechanical line leak detector as Red Jacket with model#FXIV. A copy of these an approved and <br /> current monitoring plans shall be accessible on site at all times. Immediately locate and retain a copy on site. <br /> This is a minor violation. <br /> 309 CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. <br /> The Bravo box float and chain in UDC 1/2 sump failed to stop the flow of product at the dispenser when tested. All <br /> monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the <br /> dispenser when it detects a leak. The service technician adjusted the chain(s) and verified functionality. This was <br /> corrected on site. <br /> This is a repeat violation, Class IL <br /> 323 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br /> The 87 VMI line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting <br /> a 3-gallon per hour leak at 10 psi. The service technician adjusted the leak detector, retested it, and it passed.
