The UST Monitoring Site Plan, UST Response plan was not submitted to the the California Electronic Reporting System(CERS) and the UST Letter from Chief Financial Officer did not include the facility address. Beginning January 1, 2014, all businesses are required to submit all new(or any changes to existing) UST information, including: UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Owner/Operator Written Agreement(if applicable), UST Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator Compliance online to the CERS at http://cers.calepa.ca.gov. Be sure to include any other relevant activities and required fields. This is a repeat violation, Class II. 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. Alarm logs for 2015 were not found on site. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies of 2015 alarm logs to EHD. This is a minor violation. 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. An approved and current copy of the monitoring plans were not available for inspection. The monitoring plans provided on site listed the mechanical line leak detector as Red Jacket with model#FXIV. A copy of these an approved and current monitoring plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 309 CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. The Bravo box float and chain in UDC 1/2 sump failed to stop the flow of product at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician adjusted the chain(s) and verified functionality. This was corrected on site. This is a repeat violation, Class IL 323 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. The 87 VMI line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi. The service technician adjusted the leak detector, retested it, and it passed. This was corrected on site.
