The site map marked the annular space, but did not mark <br /> the annular sensor. A site plan must be submitted identifying the locations where monitoring will be performed. <br /> Immediately log into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/and upload <br /> a copy of the UST Monitoring Site Plan. <br /> This is a minor violation. <br /> 113 CCR 2638(d) Failed to submit monitoring system certification test report within 30 days. <br /> Annual monitoring system certification, leak detector testing, and spill container testing was performed on June 25, <br /> 2015 and a test report was submitted on August 25, 2015, one month late. A copy of the test report must be <br /> submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports <br /> are performed in a timely manner. <br /> This is a minor violation. <br /> 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be <br /> accessible on site at all times. Immediately locate and retain a copy on site. <br /> This is a repeat violation, Class Il. <br /> 206 CCR 2712 Failure to comply with UST operating permit conditions. <br /> A copy of the UST operating permit was not found on site. A copy of the UST operating permit and all attachments <br /> and conditions shall be retained at the facility. Immediately obtain a copy of the permit and retain it on site. <br /> This is a minor violation. <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to attach an alarm history for the July 2015, December 2015, January 2016, <br /> February 2016, and April 2016 designated operator monthly inspection reports. The alarm histories included for <br /> November 2015, March 2016, and May 2016 did were squished against the left side of the printout, with the words cut <br /> off and illegible. During the monthly inspection, the designated operator shall review the alarm history for the <br /> previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the <br /> alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated <br /> operators performing monthly inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class ll. <br /> Page 4 of 5 <br />
