Information on contractor's insurer and worker compensation number. <br /> 2. Page 4, item 7 c of application, method for decontamination. <br /> 3. Page 5, item 9 of application, how samples will be collected. <br /> 4. Page 5, item 11 b of application, the final destination of the stockpile. <br /> 5. Page 10 of application,the tank tracking record, must be completely filled out for <br /> each tank. <br /> 6. The Acculabs is not certified to do 8260 and to perform analysis for BTEX. Please <br /> provide additional information on the laboratory. <br /> 7. Site safety officer. <br /> 8. Site map, including tank and line location and information on adjacent property. <br /> Tank Installation: <br /> 1. Tank manufacturer's installation instruction manual. <br /> 2. A site specific buoyancy calculation. <br /> 3. Cut-sheet and information on spill container. <br /> 4. The location of the new monitoring panel, page SD-3 of plan. <br /> 5. Locations for all sensors,page 6, item 14 B of application. <br /> 6. Page 8, item 19 E and F of application. <br /> 7. Contractor's worker compensation number. <br /> 8. Contractor's certificate of training issued by the tank and piping manufacturers. <br /> 9. Specify the fiberglass pipe to be used on page G-11 of plan. <br /> Please submit above information to me.
