Underground Storage Tank Program Inspection Report <br />Date: <br />April 09, 2018 <br /> Facility Address: <br /> 1257 W YOSEMITE AVE, MANTECA <br /> Facility Name: <br /> AHMEDS SONS INC <br />San Joaquin County <br />Environmental Health Department <br />1868 East Hazelton Avenue, Stockton, California 95205-6232 <br />Telephone: (209) 468-3420 Fax: (209) 468-3433 Web: www.sjgov.org/ehd <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator failed to document all the alarms from the attached alarm history on the designated <br />operator monthly inspection reports and failed to check that they were responded to appropriately. <br /> The missing alarms include: <br />On the 5/23/2018 DO inspection report, the DO, Shawn Rodriguez, did not document an L4 Diesel annular sensor <br />out alarm and an L4 Diesel annular fuel alarm that occurred on 4-28-2018. <br />On the 7/28/2018 DO inspection report, the DO, David Turner, did not print out the alarm history and did not <br />document the Q2 91 grade PLLD shutdown and gross line fail alarms that occurred on 7-26-2017. <br />During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br />that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br />documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br />monthly inspections at this facility are including all of the required information on the reports. <br />This is a Class II violation. <br /> 211 CCR 2715(f)(2) Designated operator employee training not performed or log not kept. <br />The designated operator employee training for Santos, Jasmin, Natasha, Ryan, Adrian, and Safdar is not current. <br />The training expired on 3/30/2018. The designated operator shall train facility employees for which he or she is <br />responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall <br />include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility’s best management practices <br /> 2. Employee’s role with regard to monitoring equipment as specified in the facility’s monitoring plan <br /> 3. Employee’s role with regard to spills and overfills as specified in the facility’s response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br />the training records to the EHD. <br />This is a repeat violation, Class II. <br /> 315 HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of (pre-Jul 2003). <br />Liquid was observed in the Diesel fill sump (4 gallons), 87 grade fill sump (3 gallons), and the 3/4 under dispenser <br />containment unit (2 gallons). If water could enter into the secondary containment by precipitation or infiltration, it <br />must be removed and disposed of properly. Immediately remove this liquid, make a hazardous waste determination <br />per Title 22 hazardous waste regulations, and manage it accordingly. Ensure that the fill sumps and under <br />dispenser containment units are maintained free of liquid. <br />This is a Class II violation. <br /> 319 CCR 2635(b) No spill bucket, failed to contain five gallons, or failed to provide a means to keep it empty. <br />The 87 grade spill container failed when tested. All spill containers shall have a minimum capacity of five gallons <br />and be capable of containing a spill or overfill until it is detected or cleaned up. The technicians applied pipe dope to <br />seal the container lip.
