In the event contamination is observed, confirmed or suspected as a result of a <br /> leaking UST system, it is the responsibility of the owner or operator to submit a work <br /> plan to EHD Site Mitigation Unit prior to initiating any assessment or remediation <br /> activities. <br /> b. To minimize delays, a Corrective Action Plan may be submitted concurrent with tank <br /> closure or installation plans. Address one copy of the Corrective Action Plan to the <br /> Site Mitigation Unit for review. For further assistance contact the Site Mitigation Unit <br /> at (209)468-3450. <br /> B. PLAN REVIEW AND OPERATING PERMIT FEES <br /> 1. New facility plan review fee of$1,000.00 (8 hour minimum per facility), $550.00 facility <br /> fee (includes one tank), $125.00 per tank after the first tank, plus $15.00 state <br /> surcharge fee per tank is due at time of plan submittal plus $49.00 per site state <br /> surcharge. <br /> 2. New installation of an additional tank at an existing facility is $1,000.00 pian review fee <br /> (8 hour minimum per facility) plus $125.00 per new tank after first tank, and $15.00 per <br /> new tank state surcharge fee. <br /> 3. It should be noted that a $1,000.00 plan review check fee required for each submittal <br /> allows for up to eight (8) hours of staff review and/or inspection time. Additional time <br /> spent on plan review, meetings, inspections, phone or office consultations will be billed <br /> to the applicant at a rate of$125.00 per hour. <br /> 4. The maximum review time for UST installation plans is 40 working days. A preliminary <br /> review will be conducted by the EHD within 20 working days of submittal date to <br /> identify any gross plan deficiencies. <br /> 5. If gross deficiencies are identified, the plans will be returned with a checklist identifying <br /> plan deficiencies. <br /> 6. To expedite the review of installation plans, time and a half per hour overtime fee may <br /> be submitted to have review completed within 10 working days of submittal date if staff <br /> is available to perform the review. <br /> Revised 8/1/11 2 <br />
