The service request page for the install and retrofit permits are signed by Costco. The Application For Underground Storage Tank Installation Permit is signed by Costco and Barghausen. The Application For Underground Storage Tank Retrofit or Piping Repair Permit is also signed by Costco and Barghausen. The Service Request Page and Application must all be signed by Costco or Bargausen. (Preferably Costco). Response: As discussed, we have revised both Service Request forms to indicate that Barghausen Consulting Engineers, Inc., as the Service Requestor, is the billing address. 3. Site Map T-3 Section E part 54 & 60 lists Fiberglass Systems 3" over 2" FRP. Fiberglass Systems should be changed to Ameron Dualoy. Response: The callout on Sheet T-3 has been revised to indicate Ameron Dualoy. 4. Site Map T-4 part 33 should list hydrostatic sensor-dual POINT float, with 4" NPT male adapter(for hydrostatic tank monitor). Response: The part description has been revised to indicate that it is a dual point float. 5. Site Maps that list the mechanical flapper at 95%should be changed to 90%. Response: All mechanical flapper valve callouts have been revised to indicate installation at 90 percent for overfill prevention. 6. Submit a copy of the Lubrizol compatibility letter for Blueline. Response: Please see the enclosed Manufacturer's Affirmative Statement of Compatibility prepared by Blueline Technologies dated February 7, 2013. 7. Site Maps T-1 & T-2 list the drop tube with mechanical flapper to be installed at 95% overfill prevention. Change 95% to 90%. Response: All mechanical flapper valve callouts have been revised to indicate installation at 90 percent for overfill prevention. The following outline provides each of the January 22, 2013 comments in italics exactly as written, along with a narrative response describing how each comment was addressed: 1. Does Containment Solutions have written instructions for doing the sump modification? Do they have guidelines for testing the sump modification in order to verify that the interstitial space has communication? if not, how will the contractor do both of these things? The letter states that they will complete the sump modification in accordance with the manufacturer specifications. What are the specifications? It also stated that: The installation of the new sump top will be completed such that there will be continuous monitoring within the interstitial space between all sensors. The double wall sump has only one sensor for the interstitial space. How will the contractor demonstrate that the brine from the new sump addition has communication with the existing sump? Brine could be used to show this, if the sump has a brine fill port at the bottom. Please let me know if you have any questions.
