The service request page for the install and retrofit permits are signed by Costco. The <br /> Application For Underground Storage Tank Installation Permit is signed by Costco and <br /> Barghausen. The Application For Underground Storage Tank Retrofit or Piping Repair Permit <br /> is also signed by Costco and Barghausen. The Service Request Page and Application must <br /> all be signed by Costco or Bargausen. (Preferably Costco). <br /> Response: As discussed, we have revised both Service Request forms to indicate that <br /> Barghausen Consulting Engineers, Inc., as the Service Requestor, is the billing address. <br /> 3. Site Map T-3 Section E part 54 & 60 lists Fiberglass Systems 3" over 2" FRP. Fiberglass <br /> Systems should be changed to Ameron Dualoy. <br /> Response: The callout on Sheet T-3 has been revised to indicate Ameron Dualoy. <br /> 4. Site Map T-4 part 33 should list hydrostatic sensor-dual POINT float, with 4" NPT male <br /> adapter(for hydrostatic tank monitor). <br /> Response: The part description has been revised to indicate that it is a dual point float. <br /> 5. Site Maps that list the mechanical flapper at 95%should be changed to 90%. <br /> Response: All mechanical flapper valve callouts have been revised to indicate installation at <br /> 90 percent for overfill prevention. <br /> 6. Submit a copy of the Lubrizol compatibility letter for Blueline. <br /> Response: Please see the enclosed Manufacturer's Affirmative Statement of Compatibility <br /> prepared by Blueline Technologies dated February 7, 2013. <br /> 7. Site Maps T-1 & T-2 list the drop tube with mechanical flapper to be installed at 95% overfill <br /> prevention. Change 95% to 90%. <br /> Response: All mechanical flapper valve callouts have been revised to indicate installation at <br /> 90 percent for overfill prevention. <br /> The following outline provides each of the January 22, 2013 comments in italics exactly as written, along <br /> with a narrative response describing how each comment was addressed: <br /> 1. Does Containment Solutions have written instructions for doing the sump modification? Do <br /> they have guidelines for testing the sump modification in order to verify that the interstitial <br /> space has communication? if not, how will the contractor do both of these things? The letter <br /> states that they will complete the sump modification in accordance with the manufacturer <br /> specifications. What are the specifications? It also stated that: The installation of the new <br /> sump top will be completed such that there will be continuous monitoring within the interstitial <br /> space between all sensors. The double wall sump has only one sensor for the interstitial <br /> space. How will the contractor demonstrate that the brine from the new sump addition has <br /> communication with the existing sump? Brine could be used to show this, if the sump has a <br /> brine fill port at the bottom.
