2. PLAN REVIEW AND OPERATING PERMIT FEES <br /> a. New facility plan review fee of $624.00 (8 hour minimum per facility), $170.00 fee per <br /> ko tank, plus $8.00 state surcharge fee per tank is due at time of plan submittal plus $18.50 per <br /> ,{ site state surcharge. <br /> b. New installation of an additional tank at an existing facility is $624.00 plan review fee (8 <br /> 1 hour minimum per facility) plus $170.00 fee per new tank, and $8.00 per tank state <br /> surcharge fee plus $18.50 per site state surcharge. <br /> C. It should be noted that a $624.00 plan review check fee required for each submittal allows <br /> for up to eight (8) hours of staff review and/or inspection time. Additional time spent on <br /> plan review, meetings, inspections, phone or office consultations will be billed to the <br /> applicant at a rate of $78.00 per hour. <br /> d. The maximum review time for UST installation plans is 40 working days. A preliminary <br /> review will be conducted by the PHS-EHD within 20 working days of submittal date to <br /> identify any gross plan deficiencies. <br /> e. If gross deficiencies are identified, the plans will be returned with a checklist identifying plan <br /> deficiencies. <br /> f. To expedite the review of installation pians, time and a half per hour overtime fee may be <br /> submitted to have review completed within 10 working days of submittal date if staff is <br /> available to perform the review. <br /> 3. PLAN REVIEW GUIDELINES <br /> a. The enclosed plan review sheet is to be completed and submitted with the three sets of <br /> drawings to PHS-EHD. <br /> b. A copy of the California UST Regulations may be obtained from the California State Water <br /> Resources Control Board at (916) 227-4303. <br /> C. Permits and/or plan check approval may also be required by the San Joaquin Valley Unified <br /> Air Pollution Control District, the County/City Building Department and the Fire District in <br /> your respective area. <br /> d. A minimum of 3 inspections will be conducted on the UST system installation. A $78 per <br /> hour inspection fee shall be charged for additional inspections or reinspection as required. <br /> (See required inspections below) <br /> e. Applicant/contractor to adhere to the approval plan and any change or deviation shall be <br /> reported to and approved by PHS-EHD prior to installation. <br /> f. The application for installation of underground tanks is only valid for the calendar year in <br /> which it has been issued. A permit may be extended into the next calendar year if a letter is <br /> sent to PHS-EHD requesting this extension thirty days prior to the end of the calendar year. <br /> 2 <br />
