The removal plan is incomplete . The plan needs to be <br /> resubmitted <br /> Installation Plan: <br /> 1 . Due to the high ground water the tank buoyancy needs to be <br /> addressed. Provide drawings that indicate that deadman will <br /> be used, or provide overburden calculations . <br /> 2 . Plans lack manufactures information sheets. Provide for <br /> review. <br /> 3 . In order for the facility to receive the exemption from the <br /> annual line test the pump system must demonstrate positive <br /> automatic shut-down. The monitoring system must be able to <br /> kill the turbine when the probe senses a leak, malfunctions <br /> or when any part of the monitoring system is disconnected. <br /> 4 . Any changes to construction design or equipment must be <br /> approved by PHS-EHD prior to implementing the changes . <br /> 5 . The plan indicates that two different types of leak <br /> detectors will be used. The specific type and model must be <br /> called out . <br /> 6 . The plan indicates that the vent lines will have ball check <br /> valves installed.
