The DO reports <br />indicate 6/20/17 as the annual date, but the date is actually 6/21/17. During the monthly inspection, the designated <br />operator shall check that all required testing and maintenance for the UST system have been completed and <br />document the dates they were done. Ensure that designated operators performing monthly inspections at this <br />facility are including all of the required information on the reports. <br />The designated operator failed to document all the alarms from the attached alarm history on the 11 /30/17 <br />designated operator monthly inspection report and failed to check that they were responded to appropriately. The <br />missing alarms include: L13 UDC 13/14 Low liquid fuel alarm on 11/28/17 and 11/19/17. During the monthly <br />inspection, the designated operator shall review the alarm history for the previous month, check that each alarm <br />was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken <br />in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections <br />at this facility are including all of the required information on the reports. <br />This is a repeat violation, Class II. <br />309 <br />CCR 2636(0(1) Monitoring in sump / UDC failed to activate an audible and visual alarm or stop the flow product. <br />The diesel -grade STP 208 sump sensor failed to activate an audible and visual alarm when tested to ensure <br />continuous monitoring of the tank system. All monitoring equipment shall be maintained to activate an audible and <br />visual alarm or stop the flow of product at the dispenser when it detects a leak. The sensor was replaced and <br />retested during the inspection. A "UST Retrofit Verification with Inspector Already Onsite" form has been completed <br />and provided to the operator and contractor. This was corrected onsite. <br />The L11 UDC 9/10, and L12 UDC 11/12 304 brine sensor were verified to be functional and passed testing. The <br />problem with the sensors are that they were not able to go out of alarm, which needs to be addressed. All <br />monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the <br />dispenser when it detects a leak. Correct immediately by having a properly licensed, trained, and certified <br />contractor replace the failed component with a functional component (LG 113-x listed, if applicable) and obtain a <br />permit from the EHD if necessary. The monitoring system certification was completed on 6/29/18. During this time, <br />the two sensors were replaced while inspector was present. A "UST Retrofit Verification with Inspector Already <br />Onsite" form has been completed and provided to the operator and contractor.
