The 87, 91, and diesel tank information forms should list none for the vent, vapor, Sump in the fill components installed section. The diesel tank sholuld also list none for the vapor recfor the overy containment. Y the EHD. 209 CCR 2715(f) Designated operator employee training not performed or log not kept. The designated operator employee training for Rajwinder Kaur,Alexandra Herrera, and Nirmaljit Badial were not ew ones on duty at time of inspection. The designated performd ithin the last twelve months according to the training records on site. These employees were the only operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. This is a minor violation. 309 CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. The Bravo box float and chain in the#5/6 UDC Sump failed to stop the flow of diesel product at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician clean out the debris from the UDC, adjusted the chain, and verified functionality. This is a repeat violation, Class ll. Overall Inspection Comments: On site to conduct a routine inspection and witness the annual monitoring system certification testing. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 11-9-15.
