The EHD must witness the functionality of the monitoring system to ensure that <br /> monitoring has been restored to meet the requirements of California Code of <br /> Regulations Title 23 and the Health& Safety Code. <br /> 7. If concrete is broken near the UST, a tank secondary containment test must be performed <br /> by a SWRCB licensed tank tester in accordance with the manufacturer's guidelines. <br /> Results must be received by the EHD within seven days of returning the facility to <br /> operating condition. <br /> 8. If concrete is broken near the product lines, a line secondary containment test must be <br /> performed by a SWRCB licensed tank tester in accordance with the manufacturer's <br /> guidelines. Results must be received by the EHD within seven days of returning the <br /> facility to operating condition. <br /> 9. If electrical penetration is added to UDC or sump, a 24 hour lake test or its equivalent <br /> will be required to be witnessed by this office. <br /> 10. An"Underground Storage Tank Certification of Installation/Modification" form(form C) <br /> shall be submitted by the owner or the owners'agent [pursuant to(CCR)2635(d)] before <br /> this upgrade will be considered complete by EHD. <br /> 11. Permit expires 180 days from date of approval.
