The <br /> missing alarms include: L4 premium sump fuel alarm on 6/20/17. During the monthly inspection,the designated <br /> operator shall review the alarm history for the previous month, check that each alarm was documented and <br /> responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br /> alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are <br /> including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 309 CCR 2636(f)(1)Monitoring in sump/UDC failed to activate an audible and visual alarm or stop the flow product. <br /> The veeder root 208 sensor in the 87 STP sump failed to activate an audible and visual alarm when tested to <br /> ensure continuous monitoring of the tank system. All monitoring equipment shall be maintained to activate an <br /> audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The sensor was <br /> replaced and retested during the inspection. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Some liquid was observed in the 87, 91, and Diesel STP sump.According to the service technician Anthony Reyes, <br /> approximately 3 gallons of liquid total was removed prior to inspector arriving to site from the 87, 91, and diesel STP <br /> sumps. If water could enter into the secondary containment by precipitation or infiltration, it must be removed and <br /> disposed of properly. Ensure that all sumps and annular spaces are maintained free of liquid. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receipt of this report. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer(if <br /> applicable), and the Owner Statement of Designated Operator Compliance.
