San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone: (209)468-3420 Fax: (209)468-3433 Web:www.sigov.org/ehd <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> GEORGES BP MINI MART 18662 N HWY 88 , LOCKEFORD February 23, 2016 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 206 HSC 25294, 25295 Failed to record or report unauthorized release in appropriate time frame. <br /> The six gallon diesel release from dispenser 10 on 2-22-16 at 8:30 pm was not reported to the EHD by the facility <br /> operator within 24 hours and a full report was not submitted to the EHD within five working days. The spill was <br /> reported by a customer to Cal EPA. Any release from the primary containment that does not escape the secondary <br /> containment and was not cleaned up within eight hours of detection, or any release that escapes the secondary <br /> containment shall be reported to the EHD within 24 hours and a full report shall be submitted within five working days. <br /> The report shall include at least the following information: <br /> 1. Owner's or operator's name and telephone number <br /> 2. List of types, quantities, and concentrations of hazardous substances released <br /> 3. Approximate date of release <br /> 4. Date release was discovered <br /> 5. Date release was stopped <br /> 6. Description of actions taken to control and stop the release <br /> 7. Description of corrective and remedial actions <br /> 8. Methods of clean up implemented to date, and proposed clean up actions with a time schedule <br /> 9. Method and location of disposal of waste generated from release and clean up. Attach copies of any <br /> hazardous waste manifests. <br /> 10. Description of proposed methods of repair or replacement of primary or secondary containment <br /> 11. Description of additional actions taken to prevent future releases. <br /> Immediately prepare a report with the required information and submit a copy to the EHD. Ensure that any future <br /> releases are reported appropriately. <br /> This is a Class II violation. <br /> 301 HSC 25292.1 UST system is not operated to prevent unauthorized release, including spills and overfills. <br /> The dispenser 10 diesel dispensing hose is disconnected from the dispenser. On 2-22-16, a customer was pumping <br /> fuel and the hose broke. The customer stated in a Cal EPA complaint that the hose was taped together with electrical <br /> tape and broke during fueling of their vehicle. Six gallons of diesel fuel spilled on the driveway. The underground <br /> storage tank system shall be operated to prevent unauthorized release. Immediately remove the dispenser from <br /> service and have a qualified technician repair the dispensing hose. Maintain the UST equipment to prevent releases <br /> of fuel. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> This inspection report was written 3-1-16 after a non-routine inspection on 2-23-16. 1 was on site in response to a <br /> Cal EPA complaint of a diesel spill at the facility-bin 2-22-16. A Notice to Abate was written on 2-23-16, signed by <br /> the cashier on site and provided to him. Two five-gallon containers of diesel-saturated absorbent waste was <br /> generated during the diesel spill cleanup. Provide the disposal record for this waste to the EHD by 3-23-16 as <br /> required in the Notice to Abate. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> Page 4 of 5 <br />
