June 27, 2013
Dear Inspector,
101, 122, 125, 605, 703, and 812:
RECEIVED
JUN 27 2013
ENVIRONMENTAL
HEALTH DERARTMENT
Cw 101. This facility EPA ID number is inactive... I e-mailed them the form that was given to me.
122 & 125 . We did not create any hazardous waste from the year 2011 to now. The barrel that we have
is supposed to be used if we made hazardous waste. That is why we did not label it.
605. one 55 -gallon container is used for hazard waste since we do not have any yet we didn't label it. If
we do get some hazardous material in the barrel we will label it.
703. The hazardous waste emergency information is already by the phone, but was under the papers.
Now we put it in a frame so no one can remove it.
812: We were keeping them at home and we recycled them now.
106, 201, and 305
106. The monitoring plan was submitted to CERS.
201. The alarm L3 which occurred on July 15th 2012 and on September 11th, were not found on site ,
because Elite IV fixed the problem and in the presence of an inspector they cleared the data. By the time
the UST operator came, he didn't find any alarm record there and I have documents that they work
there.
305. The 91 STP sensor was changed in your presence by Elite IV Contracter. On the 18th of June.
109 and 204
109. The report was late, because construction was going on at the facility, and we got exta time from
an inspector to fix that problem.
204. He said when he appeared to inspect the alarm he could not find the alarm data and he said he can
explain to the inspector what happened.
That is how I fixed the violations and I have documents that you can check
Sincerely,
Swaran Chouhan
