The monitoring plan uploaded to California Environmental Reporting System (CERS) is not current <br />and not approved by the EHD. The monitoring and response plans must be current and approved by <br />the EHD. Log in to the CERS and update the monitoring plan by July 18, 2013. <br />201. Maintenance records for L3 alarms which occurred on July 15, 2012 and on September 11, 2012 <br />were not found on site. These records shall be kept on site for at least 3 years. Immediately locate and <br />submit the supporting documentation to the EHD (by July 18, 2013) describing how these alarms were <br />addressed. <br />305. The 91 STP sump sensor failed to activate an audible and visual alarm when tested to ensure <br />continuous monitoring of the tank system. All monitoring equipment shall be maintained to activate an <br />audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The sensor <br />was replaced by Elite IV with like for like and retested during the inspection. The "UST Retrofit <br />Verification with Inspector Already Onsite" inspection form was completed and a copy was provided to <br />the site operator. <br />Notice to Comply- Minor violation: 109 and 204: <br />109. Annual monitoring system certification, leak detector testing, and spill container testing was <br />performed on June 11. 2012 and a test report was submitted on July 23, 2012, in excess of 30 days. A <br />copy of the test report must be submitted to EHD within 30 days of the tests.
