The monitoring plan uploaded to California Environmental Reporting System (CERS) is not current and not approved by the EHD. The monitoring and response plans must be current and approved by the EHD. Log in to the CERS and update the monitoring plan by July 18, 2013. 201. Maintenance records for L3 alarms which occurred on July 15, 2012 and on September 11, 2012 were not found on site. These records shall be kept on site for at least 3 years. Immediately locate and submit the supporting documentation to the EHD (by July 18, 2013) describing how these alarms were addressed. 305. The 91 STP sump sensor failed to activate an audible and visual alarm when tested to ensure continuous monitoring of the tank system. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The sensor was replaced by Elite IV with like for like and retested during the inspection. The "UST Retrofit Verification with Inspector Already Onsite" inspection form was completed and a copy was provided to the site operator. Notice to Comply- Minor violation: 109 and 204: 109. Annual monitoring system certification, leak detector testing, and spill container testing was performed on June 11. 2012 and a test report was submitted on July 23, 2012, in excess of 30 days. A copy of the test report must be submitted to EHD within 30 days of the tests. Ensure that future test reports are submitted to EHD in a timely manner. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
