The following is an itemized list of underground storage tank violations that have not <br />been addressed for GSG GAS & MART as of June09, 2017. <br />Open violations from June 20, 2016 inspection <br />Violation #209 - Designated operator did not inspect all required items and/or the inspection reports not <br />completed. <br />The designated operator failed to check and note the correct test dates for the secondary containment on the <br />designated operator monthly inspection reports. The date listed is 7/30/15. This testing was actually done on <br />6/30/16. During the monthly inspection, the designated operator shall check that all required testing and <br />maintenance for the UST system have been completed and document the dates they were done. Ensure that <br />designated operators performing monthly inspections at this facility are including all of the required information on <br />the reports. <br />Violation #306 - Monitoring equipment is not installed or maintained to detect a leak at the earliest <br />opportunity. <br />The test boots in the 87 and 91 STP sumps were not pulled back and the Schrader valves are pointed straight up. <br />Any potential release from the primary containment into the secondary containment in the 87 and 91 piping does not <br />flow into a collection sump. All secondary containment systems shall be sloped so that all releases will flow to a <br />collection sump located at the low point of the underground piping. Immediately contact a properly licensed, trained, <br />and certified contractor to ensure that any releases in the 87 and 91 piping flows into the piping sumps under permit <br />and inspection by the SJC EHD. <br />Page 1 of 1 <br />
