UST Facility Operating Permit Application is lacking Board of Equalization (BOE) Maintenance Fee Account Numbers or BOE number is incorrect. BOE number 44043892 shows as closed on 06/12/2016 Immediately log into CERS, update the required information, and submit for review by the EHD. UST Tank Information forms for both tanks is not current in CERS. The striker plate was selected as no, the tank installation date was listed as 4/30/1998, records indicate install date was 03/26/1998, the owner operator agreement is indicated as stored at facility. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This was corrected on site. This is a Class II violation. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. A site plan must be submitted identifying the locations where monitoring will be performed. Current plan is lacking the location of the mechanical line leak detectors. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. This was corrected on site. This is a minor violation. 121 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on June 20, 2016 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by July 20 2016. This information has not been received resulting in a non-compliant status for this facility. An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. This is a Class II violation. 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and note the correct test dates for the secondary containment on the monthly designated operator monthly inspection report. The DO noted testting was done on 07/30/2015 when testing was actually conducted on 06/30/2015. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II.
