The date listed is 7/30/15. This testing was actually done on <br />6/30/16. During the monthly inspection, the designated operator shall check that all required testing and <br />maintenance for the UST system have been completed and document the dates they were done. Ensure that <br />designated operators performing monthly inspections at this facility are including all of the required information on the <br />reports. <br />This is a repeat violation, Class II. <br />306 <br />CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br />The test boots in the 87 and 91 STP sumps were not pulled back and the Schrader valves are pointed straight up. <br />Any potential release from the primary containment into the secondary containment in the 87 and 91 piping does not <br />flow into a collection sump. All secondary containment systems shall be sloped so that all releases will flow to a <br />collection sump located at the low point of the underground piping. Immediately contact a properly licensed, trained, <br />and certified contractor to ensure that any releases in the 87 and 91 piping flows into the piping sumps under permit <br />and inspection by the SJC EHD. <br />This is a repeat violation, Class II. <br />Overall Inspection Comments: <br />An inspection checklist was provided to the facility operator on the day of inspection. The EHD has written the <br />complete report which replaces the initial checklist. <br />Page 3 of 4 <br />
