The designated operator failed to check and note the correct test dates for the secondary containment on the designated operator monthly inspection reports. The date listed is 7/30/15. This testing was actually done on 6/30/16. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. The test boots in the 87 and 91 STP sumps were not pulled back and the Schrader valves are pointed straight up. Any potential release from the primary containment into the secondary containment in the 87 and 91 piping does not flow into a collection sump. All secondary containment systems shall be sloped so that all releases will flow to a collection sump located at the low point of the underground piping. Immediately contact a properly licensed, trained, and certified contractor to ensure that any releases in the 87 and 91 piping flows into the piping sumps under permit and inspection by the SJC EHD. This is a repeat violation, Class II. Overall Inspection Comments: An inspection checklist was provided to the facility operator on the day of inspection.
