One 55-gallon container, which according to Mr. Hara (the site manager) contains waste absorbent, <br /> lacked hazardous waste label. All hazardous waste containers shall be marked with the following <br /> information: <br /> 1. the words "Hazardous Waste" <br /> 2. name and address of generator <br /> 3. hazardous properties <br /> 4. physical state <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> Immediately label this container and ensure that all future containers are marked with all the required <br /> information. Since it cannot be determined how long this container has been on site, immediately <br /> contact a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of <br /> the manifest to the EHD by July 18, 2013. <br /> 703. An emergency coordinator and modified contingency plan information is lacking. There must be at <br /> least one emergency coordinator on site or on call to coordinate emergency response measures, and <br /> the following information must be posted by a phone: the name and phone number of the emergency <br /> coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br /> phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an <br /> emergency coordinator and post the required information by a phone. A form is provided that can be <br /> used for this purpose. Submit proof of correction to the EHD by July 18, 2013. <br /> 812. Failed to keep records of each shipment of universal waste. Mr. Hara stated that when there is a <br /> waste bulb at the facility the owner takes it off-site for disposal. No records of universal waste disposal <br /> ware available for review. Copies of these records must be maintained for at least three (3) years. <br /> Immediately locate missing records and maintain them on-site, also submit copies of these records to <br /> the EHD by July 18, 2013. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by July 18, 2013. <br /> Provided: <br /> 1. A list of 2013 EHD training classes; 2. Household Hazardous Waste Disposal Guide; 3.
