This facility EPA ID number is inactive... I e-mailed them the form that was given to me. <br /> 122 & 125 . We did not create any hazardous waste from the year 2011 to now.The barrel that we have <br /> is supposed to be used if we made hazardous waste. That is why we did not label it. <br /> 605. one 55-gallon container is used for hazard waste since we do not have any yet we didn't label it. If <br /> we do get some hazardous material in the barrel we will label it. <br /> 703. The hazardous waste emergency information is already by the phone, but was under the papers. <br /> Now we put it in a frame so no one can remove it. <br /> 812: We were keeping them at home and we recycled them now. <br /> 106, 201, and 305 <br /> 106. The monitoring plan was submitted to CERS. <br /> 201.The alarm L3 which occurred on July 15th 2012 and on September 11th, were not found on site , <br /> because Elite IV fixed the problem and in the presence of an inspector they cleared the data. By the time <br /> the UST operator came, he didn't find any alarm record there and I have documents that they work <br /> there. <br /> 305.
