Dr. Martin Luther King, Jr. Blvd., Stockton, CA 95206 Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> 608. CCR 66266.130 Failed to properly manage used oil and fuel filters. <br /> Bills of lading for the recycling of used oil filters for the last three years were not found on site. A copy of <br /> each bill of lading must be kept on site for at least three years. Immediately locate a copy of all bills of <br /> lading for used oil filters for the last three years, maintain them on site, and submit copies to the EHD. <br /> An inspection checklist was provided to the facility operator on the day of inspection. The EHD has written the <br /> complete report which replaces the initial checklist. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by July 20, 2016. <br /> Note: According to Mr. Singh, the technicians have taken all test waters and the site has not had any spills that <br /> require cleaning. No disposal records were found on site. <br /> Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials information <br /> online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov. Be sure to include <br /> your hazardous waste activity in the Businesses Activities section in CERS in addition to any other relevant <br /> activities and required fields. <br /> Wastes generated at this facility: Fuel contaminated liquids, fuel contaminated absorbent, used fuel filters <br /> Documents reviewed: Modified contingency plan <br /> Documents provided: Return to Compliance Certification form <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspec Received By: � Title: <br /> EHD 23-02- 3 Rev 4/19/12 /�� CONTINUATION FORM <br />
