Contact Aris Cacapit(468-3453)to arrange the following inspection(s) (a 48-hour notice <br /> is required): <br /> a. The EHD must witness the functionality of the monitoring system to ensure that <br /> monitoring has been restored to meet the requirements of California Code of <br /> Regulations Title 23 and the Health& Safety Code. <br /> b. Before the alarm history is cleared by the"cold start",a current printout must be <br /> created and available for EHD inspection. Inspection may occur during the <br /> functionality test. <br /> 6. If concrete is being broken near the UST,pipeline and tank integrity testing must be <br /> performed by a SWRCB licensed tank tester in accordance with the manufacturer's <br /> guidelines. Results must be received by the EHD within seven days of returning the <br /> facility to operating condition. <br /> 7. An"Underground Storage Tank Certification of Installation/Modification" form(form C) <br /> shall be submitted by the owner or the owners'agent (pursuant to(CCR) 2635(d)) before <br /> this upgrade will be considered complete by EHD. <br /> 8. Permit expires 180 days from date of approval.
