Ham Ln.,Lodi CA <br /> The San Joaquin County Environmental Health Department(SJCEHD)has reviewed and approved <br /> the permit to replace the diesel turbine sump,and install new penetrations for sump piping and <br /> conduits at the above referenced location. The permit has been approved with the following <br /> conditions: <br /> 1. Contact the local fire department,and the San Joaquin Valley Air Pollution Control District <br /> for applicable permits and requirements. <br /> 2. Any changes in information or scope of work must be submitted to SJCEHD prior to <br /> commencing work. All new addendums supersede the previous ones. <br /> 3. All components must be certified by an independent testing organization and bear the <br /> appropriate mark as required by the independent testing organization. <br /> 4. At least one person who holds the International Code Council(ICC)"UST <br /> Install/Retrofitting"certification and manufacturers' certificate of training for all <br /> components installed or tested must be onsite at all times when work is performed. <br /> 5. Each person installing or testing any component must hold a certificate of training from the <br /> manufacturer of that component. <br /> 6. The"Certificate of Compliance for Underground Storage Tank Installation Form C"shall be <br /> submitted by the owner or the owners' agent[pursuant to California Code of Regulations <br /> (CCR)2635 (d)]before this upgrade will be considered complete by SJCEHD. <br /> 7. If concrete is broken,all lines shall have an integrity test performed in accordance with the <br /> manufacturer.This office must receive results within 7 days of completion of work. <br /> 8. If concrete is broken above the tank pit,a tank integrity test must be performed by a certified <br /> tank tester and results received by this office within 7 days of completion of work. <br /> 9. Any hazardous waste generated must be properly disposed of pursuant to Title 22 (Hazardous <br /> Waste Regulations). <br /> 10. Contact Toua Yang at(209)468-3459 to arrange the following inspection appointment(s); <br /> a 48-hour notice for inspection appointment(s)is required. <br /> a. 24 hour or equivalent lake test on secondary containment to verify tightness. <br /> 11. Permit expires 90 days from the approval date.
