The permit <br /> has been approved with the following conditions: <br /> 1. Contact the local fire department,and the San Joaquin Valley Air Pollution Control District <br /> for applicable permits and requirements. <br /> 2. Any changes in information or scope of work must be submitted to SJCEHD prior to <br /> commencing work. All new addendums supercede the previous ones. <br /> 3. At least one person who holds the International Code Council(ICC)"UST Service <br /> Technician"certification and manufacturers' certificate of training for all components <br /> installed or tested must be onsite at all times when work is performed. <br /> 4. Each person installing or testing any component must hold a certificate of training from the <br /> manufacturer of that component. <br /> 5. The"Certificate of Compliance for Underground Storage Tank Installation Form C"shall be <br /> submitted by the owner or the owners' agent [pursuant to California Code of Regulations <br /> (CCR)2635 (d)J before this upgrade will be considered complete by SJCEHD. <br /> 6. If concrete is broken above the tank pit,a tank integrity test must be performed by a certified <br /> tank tester and results received by this office within 7 days of completion of work. <br /> 7. Contact Toua Yang at(209)468-3459 to arrange the following inspection appointment(s); <br /> a 48-hour notice for inspection appointment(s)is required. <br /> a. If concrete is broken over the tank,corrosion protection of the risers must be verified <br /> prior to backfilling around the spill buckets. <br /> b. Spill bucket: verify minimum 5-gallon capacity and pass a 60-minute lake test or <br /> equivalent. Proper drainage must also be verified. <br /> c. EHD must verify the functionality of the overfill prevention system to ensure that <br /> monitoring has been restored to meet the requirements of California Code of <br /> Regulations Title 23 and the Health&Safety Code. <br /> 8. Permit expires 90 days from the approval date.
