Ham Lane,Lodi CA <br /> Your application for a permit to repair the secondary containment failure at the above referenced address has been <br /> received and approved with the following conditions: <br /> Conditions of approval: <br /> 1. Contact the local fire department and the San Joaquin Valley Air District for any applicable permits. <br /> 2. Any changes shall be submitted to EHD for approval prior to commencing work.All new addendums <br /> supercede the previous ones. <br /> 3. All components must be certified by an independent testing organization and labeled as required by the <br /> organization.(UL,ULC,ANSI,API,ASTM,etc. See California Code of Regulations(CCR),Title 23 <br /> Chapter 16 Table B for complete list) The component must bear the appropriate mark as required by the <br /> independent testing organization. <br /> 4. At least one person who holds the International Code Council(ICC)UST Install/Retrofitting certification <br /> and manufacturers' certificate of training for all components installed or tested must be onsite when work is <br /> performed. <br /> 5. Each person installing or testing any component must hold a certificate of training from the manufacturer <br /> of that component. <br /> 6. Contact EHD for the following inspections(Allow a 48 hour notice for all inspections): <br /> a. 24 hour or equivalent lake test on secondary containment in which penetration has been repaired. <br /> 7. If concrete is broken,all lines shall have an integrity test performed in accordance with the manufacturer. <br /> Results must be received by EHD within 7 days of returning the site to operating condition. <br /> 8. If concrete is broken near the underground storage tanks,the UST's must have an integrity test performed <br /> once concrete is poured in accordance with the manufacturer's guidelines.Results must be received by <br /> EHD within 7 days of returning the site to operating condition. <br /> 9. A"Certificate of Compliance for Underground Storage Tank Installation Form C" shall be submitted by the <br /> owner or the owners'agent[pursuant to(CCR)2635(d)]before this upgrade will be considered complete <br /> by EHD. <br /> 10.
