The UST is a 2,000 gallon used oil tank.The <br /> permit to remove the UST,associated piping, and other appurtenances from the above referenced <br /> residence has been approved with the following conditions: <br /> 1. If the backfill material and soil is found to be contaminated it must be disposed of at an <br /> approved disposal location.Treatment of soil onsite can only be performed under a hazardous <br /> waste treatment permit. <br /> 2. A copy of the"Site Health& Safety Plan"must be on the jobsite for review. <br /> 3. See pages 6& 7 of the permit application for required analytical tests and number of samples <br /> to be taken and witnessed by this department. Use Form EH 23 046, Revised 07/22/10. <br /> 4. A copy of the fire department permit must be submitted at the time of removal. <br /> 5. The following information must be submitted within 30 days after the UST removal date: <br /> (a) Only analytical results that come directly from a third party California certified <br /> laboratory with affixed chain of custody will be accepted by this office. <br /> • (b) UST Tracking Record Sheet and/or Tank Hazardous Waste Manifest. <br /> (c) Tank Closure Certification Form(DTSC form 1249). <br /> (d) Hazardous Waste Manifest for piping,rinsate,residual fuel, or waste oil receipt. <br /> 6. This permit expires 180 days after date of approval. <br /> 7. The facility owner will be billed for previous years' facility and tank fees. <br /> 8. The facility owner will be billed for additional EHD staff time expended beyond 3 hour <br /> minimum permit payment per tank. <br /> 9.
