Financial responsibility documents have not been submitted t the EHD. Current financial <br /> responsibility documents are required to be submitted annually. current copy of the financial <br /> responsibility documents were received by the EHD during the i spection. Alternatively, log into CERS, <br /> upload the required information, and submit for review by the D annually. <br /> 105. UST Facility Operating Permit Application and UST Tank Information are not current in CERS. Any <br /> change of information must be updated in CERS within 30 days of the changes. Immediately log into <br /> CERS, update the required information, and submit for review by the EHD. Complete a separate tank <br /> form for each tank. <br /> 204. The designated operator failed to check and not-the correct test dates for the sacrificial anode test <br /> on the October 2012 through August 2013 designated operator monthly inspection reports. During the <br /> monthly inspection, the designated operator shall check that all required testing and maintenance for the <br /> UST system have been completed and document the dates they were done. Ensure that designated <br /> operators performing monthly inspections at this facility are including all of the required information on <br /> the reports. <br /> 205. The September 2012 designated operat r monthly inspection report was not found on site. <br /> Designated operator monthly inspection reps for the previous twelve months shall be retained on site. <br /> Locate and ensure that copies of the previous twelve months of designated operator monthly inspection <br /> reports are maintained on site.
