Secondary containment testing was performed on October 24, 2017 and a test report was submitted on December 13, 2017, one month late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a minor violation. 115 CCR 2715(a) Failed to submit to EHD notification of designated operator identification or change w/in 30 days. A new designated operator, Nicholas Graham -Lewin was hired in May 2018 and notification was not provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD within 30 days. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. This was corrected on site. This is a repeat violation, Class II. 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. Maintenance and monitoring records for the last three years were not found on site. -Secondary containment test results for October 2017 were not found on site. -Work orders were not available for the repair and retesting conducted on secondary containment systems after failures during the October 2017 testing. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class II. 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and note the correct test dates for the secondary containment testing on the designated operator monthly inspection reports. -The test date was listed as 10/9/17 on the November 2017 through May 2018 reports. -The test date was listed as 10/9/14 on the June, July, and August 2018 test reports. The testing was conducted on 10/24/17. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II.
